Not everyone likes to plan ahead, but pre-planning your funeral arrangements not only gives you peace of mind about your final wishes being honored, but alleviates some of the stress and burden your loved ones are going through as they experience the grief associated with the loss.

By planning ahead, you help prevent those disagreements by letting your family know what your wishes are and how you want your funeral service to be carried out and your business handled.

When you preplan your funeral arrangements, there are more options when it comes to funding the funeral. A preneed insurance policy, life insurance policy or other payment option can all be arranged before the time of death. At the time of death, services must be paid in full, so having options ahead of time can also reduce other stress related to losing someone.

Following the death of a loved one, there are more than 150 decisions that have to be made within the first day or two following the death. Those stressful decisions on top of the stress of grieving a loss can be eliminated by arranging your final wishes ahead of time. If those decisions are made, your loved one’s aren’t left wondering what you would have wanted and how they should proceed with funeral plans. With those decisions out of the way, your loved one can begin the healing process sooner.

  • What steps are involved in prearranging a funeral?

    The first step would be to write down your wishes. Indicate if you would prefer burial or cremation. Write down your ideas for the service you would like, cemetery choice, etc. If you've made those choices, or even if you're unsure, schedule a time to meet with one of our funeral directors who can explain the different service options, merchandise choices, as well as the different ways to pay for the funeral.

  • What information do I need to prearrange a funeral?

    You'll need basic information about yourself or the person you are planning for, such as date of birth, marital status, parent's full names, etc. Besides the biological information, you'll need to choose burial or cremation, what type of service you'd like, what cemetery to use, etc. Our caring staff will help you with this process.

  • Do I have to pay in advance?

    Preplanning is merely making your wishes known; however, most families who do this choose to pre-fund their funeral as well. Doing so will relieve the financial burden on your family and offers peace of mind to you.

  • Can I transfer my plans to or from another funeral home?

    If you have plans at another funeral home and would like to move them here, we will help you do that. If you move from the area and would like to transfer your arrangements to another funeral home, we will assist you with that as well.

BENEFIT AMOUNT: The insured child/grandchild/great-grandchild's death benefit is the death benefit on the Policy to which the child rider is attached at the time of his/her death only until his/her 21st birthday.


ELIGIBILTY: I hereby certify that each of the below listed children/grandchildren/great-grandchildren is UNMARRIED, BETWEEN THE AGES OF 30 DAYS AND 21 YEARS, and in GOOD HEALTH (including but not limited to all medical conditions that are considered likely to result in death prior to the age of 21).


ADDING ADITIONAL CHILDREN: I understand that any additional child/greandchild/great-grandchild that I wish to cover under this RIDER must be added upon my request in writing to the Company and by my completing and executing a form provided by the Company for this purpose. I understand that sais additional child/grandchild/great-grandchild will not be covered under this RIDER until the updated, completed and executed form is received by the Company in its home offices and acknowledged in writing to the Policyholder.


NO DUPLICATE COVERAGE: No coverage can be provided under this RIDER for any child/gradnchild/great-grandchild that is covered by any other LIFE INSURANCE POLICY or CHILD RIDER written with the Company and in force at the time of the death of the child.

PET CREMATION

Upon receipt and acceptance of this Application by our office, Family1 will issue a PET CREMATION CERTIFICATE providing a reimbursement benefit to the OWNER in the AMOUNT of TWENTY-FIVE DOLLARES ($25.00) toward the cost of cremation of the remains of the deceased DOG or CAT named below.

TERMS AND CONDITIONS of the PET CREMATION CERTIFICATE:


1. The benefit is limited to reimbursement for actual cremation services, not to exceed twenty-five dollars ($25.00).

2. Only one (1) reimbursement benefit may be paid under this CERTIFICATE.

3. A maximum of one (1) CERTIFICATE is valid during the period in which the Preneed Funeral Contract named above is in effect.

4. The OWNER listed below must be the owner of the dog or cat at the time of death of the pet.

5.This CERTIFICATE is only valid if the Preneed Funeral Contract named above is in full force and effect at the time of the death of the pet.

6. The reimbursement benefit will be paid to the OWNER only upon presentation to Family1 of an invoice for creamtion of the pet by the duly licensed professional entity.

7. This benefit offered under this Certificate is not an insurance product.

Irrevocable Assignment of Policy Rights


(Relating to Pre-Arranged Funeral Goods and Services)


THEREFORE, you and I understand and agree to the following terms and conditions so that I may qualify for, or continue to be eligible for, public assistance, in accordance with applicable State or Federal law:

1. I hereby irrevocably waive any right I may have during my lifetime

(a) to surrender the life insurance or annuity contract for cash; or

(b) to obtain a loan against the life insurance or annuity contract.

2. It is understood, however, that although I have irrevocably waived my right to any cash surrender or loan value

under the life insurance or annuity contract assigned and have committed all death benefits to the payment of funeral

expenses, (a) I, and my personal representative or my next-of-kin, still retain those rights existing in the original

Contract to change the funeral home designated in the Contract and to select another funeral home to receive the

death benefits upon the death of the insured or annuitant named in the life insurance or annuity contract, in return for

the delivery of comparable funeral goods and services; and/or (b) upon my death, my personal representative or

next-of-kin retain those rights existing in the original Contract to cancel the Contract before the funeral goods and

services are provided.

3. It is further understood that you and I still have obligations under the original Contract, and all of the provisions of

that agreement which are not amended by this agreement are still in full force and effect.

4. This irrevocable assignment is made solely to facilitate my eligibility for benefits and/or assistance under Title XIX,

Social Security Act (42 U.S.C. Section 1396 et seq.) or other law providing for a public assistance program.
